معلومات هامة عن استعمال مونتيلوكاست Montelokast في علاج الربو الشعبي Asthma

اول حاجة نتكلم عنها هي تعريف الربو الشعبي او Asthma

*Asthma is defined as a chronic inflammatory disorder of the airways in which many cells and cellular elements play a role, in particular, mast cells, eosinophils, T- 
lymphocytes, neutrophils, and epithelial cells
* In susceptible persons, this inflammation causes recurrent episodes of wheezing
breathlessness, chest tightness, and cough, particularly at night and in the early morning.
*These episodes are usually associated with widespread but variable airflow obstruction that is often reversible either spontaneously or with treatment.
*The inflammation also causes an increase in the existing bronchial hyperresponsiveness to a variety of stimuli.

ازاي بيشتغل؟؟

*In asthma, leukotriene-mediated effects include airway edema, smooth muscle contraction, and altered cellular activity associated with the inflammatory process.

*في الربو الشعبي تتسبب leukotrienes  في انقباض المجاري التنفسية

*و ده اللي بيشتغل عليه montelokast

*Montelukast is an orally active compound that binds with high affinity and selectivity to the CysLT1 receptor (in preference to other pharmacologically important airway receptors, such as the prostanoid, cholinergic, or β-adrenergic receptor). Montelukast inhibits physiologic actions of LTD4 at the CysLT1 receptor without any agonist 
activity.

*بيرتبط بالمركبات اللي بتسبب انقباض المجاري التنفسية (LTD4) و بيمنع تأثيرها

*Montelukast causes inhibition of airway cysteinyl leukotriene receptors as demonstrated by the ability to inhibit bronchoconstriction due to inhaled LTD4 in 
asthmatics.

 الجرعة:

للكباراكبر من 15 عام .. قرص 10 مج مرة واحدة مساءا
للاطفال من 6 حتي 14 عام.. قرص 5 مج مرة واحدة مساءا
للاطفال من 2 حتي5 سنوات..قرص او كيس 4 مج مرة واحدة مساءا

تحذيرات الاستعمال:

يفرز في لبن الام عشان كده بلاش نستعمله في حالات الرضاعة
 لذلك  يمكن استعماله في الحمل في للضرورة القصوي Category B

الاعراض الجانبية:
  • upper respiratory infection 
  • fever  
  • headache  
  • sore throat  
  • cough  
  • stomach pain  
  • diarrhea  
  • earache or ear infection  
  • flu  
  • runny nose  
  • sinus infection
